Thank you for considering insurance coverage with Lone Star Risk Retention
Group, Inc. The process to become an insured is a six (6) step internet based
process, which will require you to read and accept the terms of the Company.1

1Lone Star Risk Retention Group, Inc. has the right to reject an
application for any reason. In addition, no coverage may be bound until all
required information requested by the Company is received by Lone Star Risk
Retention Group, Inc. Copies of all disclosure documents that must be accepted
by the potential insured are provided below.

requires potential insureds to review and sign the terms outlined in the
following disclosure documents:

Information Circular:
Outlines the technical aspects of the organization and the risks involved, read
it carefully.

Subscription Agreement:
Sets out the Company's terms for purchasing stock and your acceptance of the
conditions. Once this document is reviewed and signed; a check in the
amount of $55 per licensed bed located within the facility to be insured must
be sent to Lone Star Risk Retention Group, Inc. All checks must be made
payable to Lone Star Risk Retention Group, Inc.

Voting Agreement and Irrevocable Proxy:
Authorizes a Proxy to vote your shares at its discretion regarding the
operations of the Company.

|
|
The Lone Star Risk Retention Group, Inc. has approved only three (3) insurance
agents to market its plan. During the application process, a potential insured
must choose one of the three agents to represent them. These agents and their

required documents/information requested by Lone Star Risk Retention Group,
Inc. is to be mailed to the applicant's chosen agent (listed above). No
insurance may be bound until all of the items listed above are complete and
approved by the Company.
