Thank you for considering insurance coverage with the National Assisted
Living Risk Retention Group, Inc. The process to become an insured is a
two (2) step internet based process, which will require you to read and
accept the terms of the Company.1 This is the first step of the application
process, which will allow you to obtain a quote. To begin the process in

1National Assisted Living Risk Retention Group, Inc. has the right
to reject an application for any reason. In addition, no coverage
may be bound until all required information requested by the
Company is received by National Assisted Living Risk Retention
Group, Inc. Copies of all disclosure documents that must be
accepted by the potential insured are provided below.

This section is for informational purposes only and relates to
section 2 of the application process (section 2 requires a
username and password which will be located on your quote).
To begin the application process, please go to section I above.
The National Assisted Living Risk Retention Group, Inc. requires
potential insured's to review and sign the terms outlined in
the following disclosure documents:

Information Circular:
Outlines the technical aspects of the organization and the risks involved, read
it carefully.

Subscription Agreement:
Sets out the Company's terms for purchasing stock and your acceptance of the
conditions. Once this document is reviewed and signed; a check in the
amount of $35 per licensed bed located within the facility to be insured must
be sent to the National Assisted Living Risk Retention Group, Inc.. All checks
must be made payable to National Assisted Living Risk Retention Group, Inc. As
outlined in this Agreement, the subscription requires payment of the $35
per bed per year for three (3) years.

Voting Agreement and Proxy: Authorizes
a Proxy to vote your shares at its discretion regarding the operations of the
Company.

The National Assisted Living Risk Retention Group, Inc. has approved only

insurance agent(s) to market its plan. During the application process, a
potential insured must choose one of the

agent(s) to represent them. These agents and their contact information are
provided below:

Applications are submitted electronically (via this internet site), while all
required documents/information requested by National Assisted Living Risk
Retention Group, Inc. is to be mailed to the applicant's chosen agent (listed
above). No insurance may be bound until all of the items listed above are
complete and approved by the Company.
